Diabetes Awareness & Outreach Program
Diabetes is a condition that happens when your blood sugar (glucose) is too high. It develops when your pancreas doesn’t make enough insulin or any at all, or when your body isn’t responding to the effects of insulin properly. Diabetes affects people of all ages. Most forms of diabetes are chronic (lifelong), and all forms are manageable with medications and/or lifestyle changes.
Diabetes is a condition that can seem confusing or even a little scary. Hearing that you or a loved one has been diagnosed with this condition can bring up a lot of questions and emotions. All of us in the Lilavati Foundation Diabetes Awareness Program understand this, and we are here to help guide you through your journey with understanding diabetes and becoming a healthier you.
Objectives:
- To reduce the prevalence of diabetes and improve the quality of life of those diagnosed with type 1 and type 2 diabetes
- Health promotion through behavior change with involvement of community, civil society, community based organizations, media etc.
- Opportunistic screening at all levels in the health care delivery system from sub- centre and above for early detection of diabetes Outreach camps are also envisaged.
- To prevent and control chronic Non-Communicable diseases-Diabetes
Our Foundation Objectives:
- Educate communities about diabetes and prediabetes.
- Develop community environments that inspire and support healthy lifestyles.
- Increase access to diabetes care, medication and diagnostic equipment.
- Increase coordination and support in the implementation of national diabetes policies and plans.
Working of diabetes care and prevention in india:
- Delivery system design (moving from a reactive to a proactive care delivery system where planned visits are coordinated through a team-based approach)
- Self-management support
- Decision support, particularly at the point of care during a clinical encounter (basing care on evidence-based, effective care guidelines)
- Clinical information systems (using registries that can provide personspecific and population-based support to the care team)
- Community resources and policies (identifying or developing resources to support healthy lifestyles)
- Health systems (to create a quality-oriented culture)
